Athletic Clearance
All athletes must be registered on Athletic Clearance before they can participate in a Vista sport (try-out, pre-season conditioning, or clinic (including summer). See below for the link. We are asking all families to upload their child's PHYSICAL FORM to their athletic clearance account. Please email your coach if you are not able to upload your forms. As a part of the clearance process every athlete is required to have a yearly physical from a qualified physician. Physicals are good for one year.

Already have an Account?
If you have already created an Athletic Clearance account (same account used for FMS or SMS athletes), log back into the account and Start a NEW Clearance: choose the School Year, School and Sport.
What needs to be turned into the coach to complete my athlete's registration?
Once you have registered your athlete, no other forms need to be turned in to your coach unless the coach is requesting additional forms.
Your athlete will be cleared to play once your clearance is in good order.
